• AUTHORIZATION FOR SERVICES

    Thank you for choosing veterinarian, grooming, boarding or other services for your pets with our Clinic! By utilizing these services, you are agreeing to the following terms:
    1. Terminology:
    a. Any reference to “Clinic” refers to P&L Keller, LLC (d/b/a Compassionate Care Veterinary Clinic) (d/b/a The Ruff House) (d/b/a S’paw).
    b. Any reference to “you” refers to the individual signing below. Said individual indicates that he or she is the sole owner of the pet. (If another individual co-owns the pet or if there is a dispute regarding ownership of said pet, you must notify the Clinic in writing prior to obtaining any services for the pet).
    c. Any reference to “pet” refers to any animal(s) that you bring to the Clinic for services at any time. (If you bring an animal to the Clinic that is not your pet, you must notify the Clinic in writing prior to leaving said animal at Clinic so that the proper Authorization can be given by the appropriate owner).

    2. Scheduled Services: For services that you have scheduled in advance, you may request an estimate of fees prior to your appointment. While the Clinic does its best to provide the most accurate estimate possible, you understand that an estimate may vary from the invoiced amount due to a variety of factors unknown prior to services being provided. The Clinic will do its best to keep you up to date regarding possible deviations from the estimate.

    3. Urgent Services:
    a. If your pet encounters an urgent problem while in the Clinic’s care, the Clinic will use its sole discretion to perform services to ensure your pet remains in the best health possible. You agree not to hold the Clinic liable for such decisions made in the Clinic’s sole discretion.
    b. Urgent problems include but are not limited to a) life-altering emergency services, b) time-sensitive procedures, c) sanitation services requiring bathing or grooming, d) parasite treatment, and e) vaccine treatment (which may also include a full examination).
    c. Clinic will use best efforts, given the situation, to contact you prior to providing any services for urgent problems but you authorize the Clinic to use any means that it in its sole discretion deems appropriate.

    4. Vaccinations: You will provide the Clinic with your pet’s vaccination records (including any updates) and will abide by the Clinic’s vaccination and health requirements. This includes maintaining current Rabies, Distemper, Bordetella vaccinations and a current fecal test.

    5. Fees: You agree to pay the invoices provided by the Clinic upon receipt and prior to removing your pet from the Clinic. This includes invoices for scheduled services provided and also urgent services provided, regardless of whether such urgent services were preapproved by you. Services will be billed at the Clinic’s prevailing current rates at the time of services. Any invoices unpaid after 30 days will incur a 1.5% interest charge.

    6. Liability:
    Clinic will exercise due diligence and care in providing services to your pet.
    a. Damages Caused To Pet: You release the Clinic (including its agents, employees and owners) from any and all liability of any nature, for injury and damage, caused to your pet. This includes but is not limited to any damages related to your pet escaping from the Clinic’s fenced yard and any damages caused by unforeseen situations such as fire or natural catastrophe.
    b. Damages Caused By Pet: You agree to indemnity and hold Clinic harmless for any injury or damage caused by your pet to Clinic (including its agents, employees and owners) or to third parties.
    c. Abandoned Pet: If you fail to pick up your pet after five calendar days from the stated discharge date, Clinic will assume the pet has been abandoned and will abide by required legal steps for abandoned pets.
    d. Personal Items: Clinic requests that you do not leave personal items (such as toys and blankets) with your pet. However, if you choose to do so, Clinic is not responsible if the items become lost, broken, or soiled.
    e. Yard: Your pet will periodically be taken outdoors in a securely fenced yard. If your pet has a history of escaping fenced yards, you must notify the Clinic in writing.

    7. Duration:This Agreement shall continue until either party provides a written, dated notice of termination to the other party. However, the provisions in Paragraphs 5 and 6 survive termination. This Agreement shall replace any prior agreements relating to any pet between you and the Clinic.
